MANDATORY DOCUMENTS FOR MEDICAL VISA APPLICATION for DELHI

 

  1. Manually filled visa application form by blue or black pen in capital letters with 1 photograph affixed to it, signed by the applicant
  2. Original Passport

- The original passport should be issued less than 10 years with a minimum validity of 6 months from the date of entry to Turkey.
- Your passport must have at least 2 blank pages.
- The photocopy of valid passport first and last pages.

  1. Biometric Photographs

- Two biometric photos with white background that were taken within the last 6 months, depicting the full face including ears and all hair, clearly and completely visible without shadow, in 2.5 inch x 2.5 inch size.

  1. Travel Health Insurance

- Travel insurance should cover at least duration of the trip in Turkey

  1. Invitation Letter

- Invitation letter from the respective medical institute in Turkey specified the full name and passport number of the applicant (original or colour copy)
- Letter from the respective Doctor(s) of Donor and Recipient which should mention the medical history of both on respective clinic’s letterhead.
- Personal covering letter from the Donor & Recipient stating the purpose of visit to Turkey including the duration of stay, expenses clause and guarantee clause

  1. If Donor or Recipient is a working professional, then original leave sanction letter on the company letterhead with authorized name and signatory.
    - If Self-employed or a Businessman or a Proprietor of a firm then the applicant should give covering letter on firm letterhead and must attach business proof like Company Registration (Attested by Notary & Mantralaya) or Recommendation letter from the relevant Chamber of Commerce.
    If applicant is financially supported,
    - If Applicant has not enough funds or bank account, financial support letter from the sponsor notarized (Not applicable for spouses, parents and children)
  2. Proof of financial means

- Bank Statement for last 3 months with sufficient balance.

- Confirmed reservation of round-trip flight ticket
- Hotel Reservations

  1. Mandatory documents for the Minors

- Minors need consent letter from both parents in all cases (eventhough minor is travelling with both parents). The consent NOC must be on stamp paper with photos of parents and applicant and notarized

  1. Additional Documents
    -If Agent is making the application on behalf of the applicant, letter of attorney from every applicant, in original.
  2. Personal covering letter from the donor stating his free will to donate (attessted by notary
  3. Personal coverin letter from the recipint stating purpose of visit (attested by notary)

IMPORTANT NOTE FOR DELHI: All submitted documents must be current and complete. The incomplete submitted for the visa application in Turkey may cause the application to be rejected. Embassy of Turkey in New Delhi has the right to request additional documents when it deems necessary. Presenting of the above documents does not guarantee that the visa will be granted. The Embassy reserves the right to refuse application without any explanation. There is no refund possibility in any circumstances. MANDATORY DOCUMENTS FOR MEDICAL VISA APPLICATION FOR MUMBAI

 

(Jurisdiction areas of the Consulate General in Mumbai/Visa application centers: Mumbai, Pune, Ahmedabad, Bangalore, Chennai, Cochin, Trivandrum, Pondicherry and Goa) 

 

1.Original Passport

-Passport must be valid for at least 6 months as from the date of arrival in Turkey.

-Passport should have at least 2 blank pages available.

-1 photocopy of passport’s first and last page.

-For passports which were not issued in the jurisdiction areas of the Consulate General in Mumbai, local address proof must be submitted. (Only registered leave and license agreement copy or registered agreement copy)

 

2.Visa Application Form

-Application Form should be filled manually by blue or black pen in capital letters with 1 photograph affixed and signed by the applicant.

 

3.Biometric Photograph

-2 biometric photos with white background that were taken within the last 6 months depicting the full-face including ears and hair, clearly and completely visible without shadow, in 2.5-inch x 2.5-inch size.

 

4.Cover letter

-Letter by the applicant mentioning the purpose and duration of stay, accompanying persons if any, etc with expenses/guarantee clause. 

 

5.Hotel reservation

-Reservation document with applicant’s name and check-in & check-out dates.

 

6.Round trip flight ticket or confirmed reservation

 

7.Letter from local hospital/medical institute (with letterhead, signature, seal/stamp mentioning the medical history of the applicant.)

 

8.Invitation/acceptance letter from hospital/medical institute in Turkey (with letterhead, signature, seal/stamp.)

 

9.Travel Health Insurance 

It should fulfill the below requirements:

-Medical evacuation and repatriation expenses
-Personal accidental death
-Personal liability
-Daily allowance in hospitalization cases

 
This plan should cover the insured for a minimum of Euro 30000.
The travel insurance should cover the duration of stay in Turkey. 

 

10.Mandatory documents for minors 

 

-Consent letter from both parents even if minor is travelling with both parents. 
-NOC must be on 100Rs stamp paper with photos of parents and applicant & notarized.

-Original Passport from both parents even if minor is travelling with both parents.

 

11.Original authorization letter

-It is required from the applicant and travel agent in case application is submitted by a representative or travel agent.

 

12.Additional documents:

-(If not shown in passport) Marriage Certificate of the Couple issued by the marriage registrar /Affidavit required attested by Notary Public.

-E-visa copy and passport copy of those who will travel with the applicant should be submitted as applicable.

 

13.Proof of profession & financial means (as applicable)

-Leave/cover letter from employer/company with letterhead, signature, seal/stamp.

-Company registration document (Original notary and Mantralaya/MEA attestation needs to be done for each applicant)

-Letter from the sponsor and last 3 months' personal bank statement of the sponsor (original or copies attested by bank)

-Last 3 months’ salary slips with letterhead, signature, seal/stamp
-Last 3 months’ personal bank statement (original or copies attested by bank)

2.In principle, visa applications should be made:

 

-Minimum 10 working days prior to the date of departure (Mumbai)

-Minimum 12 working days prior to the date of departure (Other visa application centers under the jurisdiction of the Consulate General in Mumbai)

 

Announcement on visa applications:

We encourage everyone considering travel to Turkey to apply for visa as early as possible. Early visa applications are important as the processing time may vary depending on the number of applications. Visa applications are processed as efficiently as possible, however a high demand for visas can lead to a longer processing time. It is best to wait until after you have received your visa before booking travel plans.

 

Holders of Turkish visa are kindly requested to check their visa including the “duration of stay”, “period of validity” and “number of entries” before their travel to Turkey. Holders of Turkish visa will not be allowed to enter Turkey before the start of the “validity period” of their visas.


The information printed on issued visas should match the information in the applicant’s passports. You should check your visa carefully, and if you discover an error (misspelling, error in passport no. or name-surname etc.) on your visa, please email or call us.

  1. Original passport (with at least 6 months validity; and at least two empty pages)

-Previous passport needs to be submitted if current passport is issued before 2015

  1. Visa  Application form: Manually filled visa application form by blue or black pen in capital letters with 1 photograph affixed to it, signed by the applicant
  2. 1 copy of your passport’s data page

-A4 size copy - 1st page and Last page, also attach if you have any valid visas)

  1. Travel Insurance
  2. Personal covering letter from the Donor & Recipient stating the purpose of visit to Turkey including the duration of stay, expenses clause and guarantee clause.
  3. If Donor or Recipient is a working professional then Original leave sanction letter on the Company letterhead with authorized name, signatory and company seal with 3 months payslips (with seal)
  4. If Self-employed or a Businessman or a Proprietor of a firm, then the applicant should give covering letter on firms letterhead and must attach Business proof like Company Registration (Attested by Notary) or Recommendation letter from the relevant Chamber of Commerce.
  5. Letter from the respective Doctor(s) of Donor and Recipient which should mention the medical history of both on respective clinic’s letterhead.
  6. Acceptance cum Invitation letter from the respective medical institute in Turkey.
  7. Bank Statement for last 3 months with sufficient balance (1 lac and above) with bank seal.
  8. Flight tickets.
  9. Proof of accomodation: Hotel reservation for your entire stay in Turkey
  10.  Biometric Photographs:

 According to biometric specifications, not older than 6 months

-Photo Specifications – size 50*50 mm, Full face, front view, eyes open, white background, natural expression, Height of the head between 25-35mm).

Note: After the consulate verifies the documents submitted by the applicant, the consulate holds the right to ask for additional documents depending on the case if required.
